چکیده

In revision hip arthroplasty, implant retrieval and residual bone cement removal may be major problems. Complete removal of the bone cement is key for the stable anchorage of the revision stem. As proximal access does not always provide an optimal view, fractures or inadvertent perforations of the cortical bone with the chisel or drill are frequently reported complications.1-3 Windowing of the femoral shaft facilitates the procedure and ensures greater accuracy.1,4-7 Although it may weaken the femoral shaft and thus increase the risk of postoperative femoral fractures, the revision stem should bypass the window distally by at least twice the diaphyseal width.4,6-10 We have not followed this recommendation because we believe bridging of the cortical defect by 4-5 cm is irrelevant. As our technique is controversial to the literature, we have analyzed its clinical and radiographic outcome.

Hip Arthroplasty and its Revision in a Child: Case Report and Literature Review

Juvenile idiopathic arthritis is the leading cause of hip replacement in young children. However, arthroplasty in this population is challenging with several concerns about quality of the growing bone, young age for revision surgery, and difficulties in potential several revisions. In this study we introduce a case of a 12-year old who is one of the youngest patients to undergo revision hip art...

پارامترهای آناتومیک مفصل هیپ و ارتباط آن‌ها با پارامترهای فیزیکی بدن

Background and Objective: Arthrosis of the hip joint is one of the most common complications of patients and many efforts have been made to find its etiology. One of the subjects important for researchers is morphologic parameters of the hip joint and its relationship with osteoarthritis of this joint. Since these parameters themselves are affected by physical parameters of the body and the rac...

Revision of the cemented femoral stem using a cement-in-cement technique: a five- to 15-year review.

The removal of well-fixed bone cement from the femoral canal during revision of a total hip replacement (THR) can be difficult and risks the loss of excessive bone stock and perforation or fracture of the femoral shaft. Retaining the cement mantle is attractive, yet the technique of cement-in-cement revision is not widely practised. We have used this procedure at our hospital since 1989. The st...

Reconstruction of femoral defects in revision hip surgery: risk of fracture and stem migration after impaction bone grafting.

The use of impaction bone grafting during revision arthroplasty of the hip in the presence of cortical defects has a high risk of post-operative fracture. Our laboratory study addressed the effect of extramedullary augmentation and length of femoral stem on the initial stability of the prosthesis and the risk of fracture. Cortical defects in plastic femora were repaired using either surgical me...

Interprosthetic femoral fractures: analysis of 14 cases. Proposal for an additional grade in the Vancouver and SoFCOT classifications.

INTRODUCTION Interprosthetic fracture is a rare but serious entity, impairing consolidation and stability due to adverse mechanical conditions related to bone fragility and implant volume. OBJECTIVE The present study highlights the difficulties involved in managing such fractures, details treatment options and reports findings leading to a proposed additional grade in the comparable Vancouver...
